Independent Contractor Agreement For Models
rev. 9/28/2005
This agreement, by and between The Rose Agency, hereinafter called "agency", and the model (hereinafter called "model") named in the Online Welcome Form being submitted, is effective as of the earliest date of submission as evidenced by internet file transfer records.
Model agrees that submission of the online form will be evidence of acceptance of the terms herein, and further agrees to be bound by them as if model had affixed a signature hereto.
Agency agrees to use its best efforts to solicit and procure work for model as an independent contractor.
Model will not be obligated to perform actions outside the scope of their duties, and in no case will they be required to perform any illegal actions.
Model agrees not to disclose confidential information to anyone, including rate of pay.
Model hereby agrees not to perform any work or service for any client of agent either directly or indirectly for a period of five year after model has ceased working with agent.
Model agrees to pay Agency a twenty percent (20%) commission on all modeling or other work obtained through an audition or directly booked due to the Agent’s efforts.
Model has read and understood the policies and procedures (which change periodically) and agrees to review them by going to http://net33.com/rose/policies.htm before every job/audition.
Model certifies that he/she is legally permitted to work in this country because of citizenship or other status.
Model agrees to furnish legible copies of valid ID (driver's license, passport, etc.), Social Security card, a signed W-9 form (available on our website) and proof of legal work status, as requested, by fax or email, before receiving payment for work done.
Model agrees to allow images to be used for promotional use only.
